Η αποκάλυψη των ιατρικών σφαλμάτων και η βελτίωση της ασφάλειας των ασθενών
είναι θέματα άρρηκτα συνδεδεμένα μεταξύ τους, και προβάλλουν έναν από τους
βασικότερους λόγους της αποκάλυψης και αναφοράς σφαλμάτων. Ο ιατρός που
διαπράττει ιατρικό σφάλμα, νοιώθει ντροπή, ενοχή και αίσθηση αποτυχίας. Οι ιατροί
πιστεύουν ότι η αποκάλυψη ιατρικών σφαλμάτων δημιουργούν βλάβη στη σχέση
ιατρού και ασθενή. Οι ιατροί λαμβάνουν ελάχιστη εκπαίδευση σχετικά με τον τρόπο
της κοινοποίησης στους ασθενείς τους ιατρικών σφαλμάτων. Τα ιατρικά σφάλματα
αποτελούν πολύμορφο πρόβλημα που μπορούν να συμβούν τόσο στο ιδιωτικό όσο και
στο δημόσιο χώρο της υγείας. Κύρια αιτία ιατρικού σφάλματος, θεωρείται η μη
επαγγελματική επάρκεια του ιατρού. Η πλειοψηφία των περιπτώσεων αφορούν
καθυστερήσεις ή μη πραγματοποίηση ιατρικών επεμβάσεων ή μη χορήγησης
φαρμακευτικών σκευασμάτων, χρήση ακατάλληλων εργαστηριακών εξετάσεων, μη
σκόπιμες διαδικασίες, χορήγηση λανθασμένων αγωγών και μη λειτουργών εξοπλισμός.
Οι ιατροί δεν θα πρέπει να αποφεύγουν την αναζήτηση ιατρικής συμβουλής. Επιπλέον
οι οργανισμοί παροχής υγείας οφείλουν να προσανατολίσουν το ιατρικό και το
παραϊατρικό προσωπικό στην ανάπτυξη οδηγιών και διαδικασιών, όπως επίσης
οφείλουν να προσφέρουν ευκαιρίες συνεχούς εκπαίδευσης σε ατούς. Είναι απαραίτητο
ότι οι κανόνες και οι αρχές να αντιστοιχούν στην τρέχουσα πρακτική και στο σύγχρονο
τεχνολογικό περιβάλλον, ενώ τα κατάλληλα νομικά μέτρα οφείλεται να εφαρμόζονται
ώστε να αντιμετωπίζονται τα ιατρικά σφάλματα. Η φύση του προβλήματος απαιτεί μία
συστηματική προσέγγιση προκειμένου να επιτευχθεί μείωση των ιατρικών σφαλμάτων.
Είναι σημαντικό, η νομοθετική εξουσία να περιγράφει, να αντιμετωπίζει και να
διορθώνει τα ιατρικά σφάλματα.
Abstract
Disclosure of medical errors and improvement in patient safety are closely linked, and
provide one of the strongest reasons to report and disclose errors. The physician who
commits a medical error may feel shame, guilt and a sense of failure. Physicinas believe
that disclosing medical errors will cause damage to the physician-patient relationship.
Physicians receive hardly any training in how to communicate with patients about
medical errors. Medical errors are a multifaceted problem which may occur in any
private or public health care setting. Professional competence has been targeted as the
main source of physicians’ errors. The majority of error reports involved delays or
omissions of surgical interventions or medications, inappropritae diagnostic tests,
unnecessary procedures, medication errors and malfunctioning equipment. Physicians
should not be reluctant to seek out and utilise each other as resources. In addition health
care organizations have the responsibility to develop thorough orientation for all health
care professionals and technicians, in order to develop policies and procedures and
offer opportunities for continuous staff training. It is imperative that rules and laws are
relevant to current practice and technology environment and that appropriate legal steps
are followed in order to address medical errors. The nature of the problem needs a
comprehensive approach in reducing medical errors. It is vital that legislature identifies,
addresses, and acts to eliminate medical errors.
