Διερεύνηση των αλλαγών και ανίχνευση των διαταραχών στις συνήθειες του ύπνου και της ποιότητας ζωής σε ασθενείς με νεφρική νόσο υπό αιμοκάθαρση

Εισαγωγή: Οι ασθενείς με νεφρική νόσο υπό αιμοκάθαρση φαίνεται να εμφανίζουν πτωχή ποιότητα ζωής και διαταραχές ύπνου που σχετίζονται με αυξημένη νοσηρότητα, μειωμένη τήρηση της θεραπευτικής αγωγής και εμφάνιση διαταραχών συμπεριφοράς ή συναισθήματος.
Σκοπός: της παρούσας έρευνας ήταν η διερεύνηση των αλλαγών και των διαταραχών ύπνου, καθώς και της ποιότητας ζωής των ασθενών με νεφρική νόσο υπό αιμοκάθαρση.
Μεθοδολογία: Πραγματοποιήθηκε τυχαία δειγματοληψία σε ασθενείς με νεφρική νόσο υπό αιμοκάθαρση ιδιωτικών μονάδων τεχνητού νεφρού των νομών Αττικής και Κορινθίας, με τη χρήση ανώνυμου ερωτηματολογίου, που περιελάμβανε δημογραφικά στοιχεία, την κλίμακα ύπνου Epworth, το δείκτη ποιότητας ύπνου Pittsburgh και το World Health Organization Quality of Life Questionnaire. Η ανάλυση έγινε με το στατιστικό πακέτο SPSS 17.0.
Αποτελέσματα: Το δείγμα της μελέτης αποτέλεσαν 71 ασθενείς (άντρες 78.8%, n=56), ηλικίας άνω των 51 ετών (73.2%, n=52), έγγαμοι (66.2%, n=47), βασικού και μέσου εκπαιδευτικού επιπέδου (73.2%, n=52), που δεν εργάζονταν (81.7%, n=58) και ζούσαν μαζί με τις οικογένειές τους ή φροντιστές (87.3%, n=62). Το 52.1% (n=37) του δείγματος αξιολογεί την ποιότητα ζωής του ως καλή/πολύ καλή, το 45.07% (n=32) ως μέτρια τη γενική κατάσταση της υγείας του, το 66.2% (n=47) την ποιότητα ύπνου του ως μάλλον καλή/ πολύ καλή και το 85.92% (n=61) παρουσίασε φυσιολογικού βαθμού υπνηλία τον τελευταίο μήνα. Παρατηρείται θετική συσχέτιση ανάμεσα στην ποιότητα ζωής και τον βαθμό ικανοποίησης της κατάστασης της υγείας των συμμετεχόντων (p<0.05). Η πλειοψηφία του δείγματος με καλή/πολύ καλή ποιότητα ζωής αξιολογεί θετικά την ποιότητα του ύπνου, ενώ υψηλά ποσοστά ποιότητας ζωής παρουσιάζονται και για τους ασθενείς με χαμηλό βαθμό αξιολόγησης της ποιότητας ύπνου. Όσον αφορά την ικανοποίηση που νιώθουν με την υγεία τους, μειώνεται όσο αυξάνεται το χρονικό διάστημα υποβολής θεραπείας.
Συμπεράσματα: Τα αποτελέσματα της μελέτης μας αντιτίθενται στα δεδομένα της πλειοψηφίας των διεθνών μελετών, γεγονός που ίσως οφείλεται στο μικρό δείγμα ή την κουλτούρα των Ελλήνων ασθενών και δεν επιτρέπει την εξαγωγή γενικεύσιμων 12
αποτελεσμάτων, αναδεικνύοντας την ανάγκη μιας αμιγούς επιδημιολογικής μελέτης αντιπροσωπευτικού δείγματος αιμοκαθαιρόμενων ασθενών.
Abstract
Introduction: Renal patients under dialysis seem to have poor quality of life and sleep disturbances that are linked to increased morbidity, poor treatment compliance and behavioural or emotional disorders.
Aim: The aim of the present study was to investigate changes in renal patients' sleeping habits and quality of life.
Methodology: A random sample of renal patients under dialysis was selected from private dialysis units in the regions of Attica and Corinth; an anonymous questionnaire was used that included the patients' demographics, the Epworth Sleepiness Scale, the Pittsburgh Sleep Quality Index and the WHO Quality of Life Questionnaire. The SPSS 17.0 software was used for the statistical analysis.
Results: The sample consisted of 71 patients (78.8% males, n= 56), aged over 51 years (73.2%, n= 52), married (66.2%, n= 47), of primary and secondary education (73.2%, n= 52), not working (81.7%, n= 58), living with family or caregivers (87.3%, n= 62). 52.1% of the participants (n= 37) assessed their quality of life as good/very good, 45.07% of them (n=32) thought that their general health status was moderate, 66.2% of them (n=47) reported good/very good quality of sleep, and 85.92% (n=61) had had normal levels of sleepiness in the past month. A positive correlation between quality of life and satisfaction for health status was found (p<0.05). Most of the participants who enjoyed good/very good quality of life gave high marks to their quality of sleep as well, while even the participants who had poor quality of sleep, reported good quality of life. Regarding their health status satisfaction, the longer they are under treatment, the less satisfied they feel for their general health status.
Conclusions: Our findings contrast with those of most international studies, something that could be attributed to the small sample size, or the Greek patients' cultural differences. Nevertheless, wider conclusions cannot be drawn at this point, since an epidemiological study with a larger and more representative sample is necessary for safer conclusions to be reached. 14
