Σκοπός: Η καταγραφή και διερεύνηση των αιμορραγιών ανωτέρου πεπτικού στο νομό Λακωνίας και η σύγκριση των ευρεθέντων στοιχείων με τα διεθνώς καταγεγραμμένα δεδομένα.
Υλικά και μέθοδος: Πληθυσμό της μελέτης αποτέλεσαν 367 άτομα, 257 άνδρες και 110 γυναίκες, που νοσηλεύτηκαν με οξεία αιμορραγία ανώτερου πεπτικού στο Γ. Ν. Λακωνίας από τη 1/1/2010 έως τις 31/12/2014. Πραγματοποιήθηκε συστηματική καταγραφή των δημογραφικών στοιχείων, των έξεων, του ατομικού αναμνηστικού, της κλινικής εικόνας, της αντικειμενικής εξέτασης, του παρακλινικού ελέγχου και της πορείας νόσου των ασθενών.
Αποτελέσματα: Η μέση επίπτωση της νόσου διαμορφώνεται στα 82,34 περιστατικά ανά 100.000 πληθυσμού και η θνητότητα ανέρχεται στο 5,7% (21/367). Ηλικιακά, οι ασθενείς είναι άνω των 65, με τη μέση ηλικία για τους άνδρες να είναι 68,62 έτη, για τις γυναίκες 77,45 έτη και την ηλικιακή διαφορά μεταξύ των δύο φύλων στατιστικώς σημαντική (p< 0.005). H ενδοσκόπηση πραγματοποιείται στις 3.79 ημέρες από την εισαγωγή, με τα ευρήματα αυτής κατά σειρά συχνότητας να είναι ο συνδυασμός διαβρώσεων-έλκους (34,6%), το πεπτικό έλκος (21,9%), οι γαστροδωδ/κές διαβρώσεις (15%). Το ποσοστό επαναιμορραγίας διαμορφώθηκε στο 2,7%(10/367), ενώ η μέση διάρκεια νοσηλείας στις 6,89 ημέρες.
Συμπεράσματα: Η οξεία αιμορραγία ανώτερου πεπτικού στο νομό Λακωνίας εμφανίζει επίπτωση μικρότερη από τα διεθνώς παρατηρούμενα και θνητότητα που κινείται στις χαμηλότερες καταγεγραμμένες τιμές. Αποτελεί νόσο των ηλικιωμένων, ο χρόνος της ενδοσκοπικής της αντιμετώπισης υστερεί από τη διεθνή καλή πρακτική, ο μέσος χρόνος νοσηλείας κινείται εντός ορίων, ενώ εμφανίζει υποπολλαπλάσιο των διεθνών ποσοστό υποτροπής.
Abstract
Purpose: The investigation of acute upper gastrointestinal hemorrhage in the Prefecture of Laconia and the comparison of findings with the internationally recorded data.
Materials and Methods: All 367 patients (257 male and 110 female), who were hospitalized from 1/1/2010 until 31/12/2014 with acute upper gastrointestinal hemorrhage in Laconia General Hospital, were included in the study. Information concerning patients’ demographic data, habits, risk factors, medical history and comorbidities, as well as data related to clinical presentation and physical examination, diagnostic tests, treatment and clinical outcome was gathered from their medical records.
Results: Acute upper gastrointestinal bleeding had an incidence rate of 82.34 persons per 100,000 residents and mortality rate of 5.7% (21/367). Most of the patients were older than 65 years old, with the average age for men being 68.62 years and for women 77.45 years. Furthermore, this age difference between sexes was found to be statistically significant (p < 0.005). Endoscopy was performed at 3.79 days from admission, and its findings in order of frequency were the combining of erosions with ulcer (34.6%), peptic ulcers (21.9%) and erosions (15%). The rebleeding rate stood at 2.7% (10/367), while the average length of hospitalization found to be 6.89 days.
Conclusions: The incidence rate of acute upper gastrointestinal hemorrhage in Laconia is lower than the internationally observed and the mortality rate is at the lowest recorded values. The disease concerns mainly the elderly, the time of endoscopic treatment is far from the international good practice, the average length of hospitalization is within limits, while rebleeding rate is aliquot part of the international recurrence rate.
