Ο ξαφνικός θάνατος κατά την διάρκεια αθλητικών δραστηριοτήτων, αποτελεί ένα σχετικά σπάνιο, αλλά ταυτόχρονα τραγικό γεγονός, με σημαντικές οικονομικές και κοινωνικές συνέπειες. Σύμφωνα με τις υπάρχουσες μελέτες, η ετήσια συχνότητα του κυμαίνεται από 3/100 000 έως 1/250 000 . Τα θύματα αφορούν κυρίως τον ανδρικό πληθυσμό, σε αναλογία εννέα άνδρες προς μια γυναίκα. Οι έγχρωμοι αθλητές είναι αναλογικά πιο επιρρεπείς και παρουσιάζουν μια ιδιαίτερη ευαισθησία στην υπερτροφική μυοκαρδιοπάθεια. Οι αθλητές παρουσιάζουν 2,8/1 μεγαλύτερο κίνδυνο να υποστούν ξαφνικό θάνατο από ότι οι μη αθλητές. Όμως το γεγονός αυτό συνήθως οφείλεται στην μη αντιληπτή εκ των προτέρων, υποκείμενη καρδιαγγειακή νόσο.
Η κυριότερη αιτία ξαφνικού καρδιακού θανάτου για τα άτομα μεγαλύτερα των 35 ετών, με μεγάλη διαφορά από τη δεύτερη, είναι η στεφανιαία νόσος. Όσον αφορά τα άτομα κάτω των 35 ετών, οι απόψεις διίστανται με τις μελέτες που αφορούν τις Η.Π.Α. να έχουν ως πρώτη αιτία την υπερτροφική μυοκαρδιοπάθεια, ενώ μελέτες στην Ιταλία, την αρρυθμογόνο καρδιοπάθεια της δεξιάς κοιλίας. Άλλες συχνές αιτίες ξαφνικού θανάτου κατά την άθληση είναι οι συγγενείς ανωμαλίες των στεφανιαίων αρτηριών, το Commotio Cordis, η μυοκαρδίτιδα, η διαστολική καρδιομυοπάθεια, η πρόπτωση της μιτροειδούς βαλβίδας, η στένωση της αορτής, η ρήξη της αορτής κλπ. Από τους μη καρδιακούς ξαφνικούς θανάτους, το αμβλύ τραύμα αποτελεί την συχνότερη αιτία και τα μέρη του σώματος που γίνεται συχνότερα το κτύπημα, είναι το κεφάλι και ο λαιμός. Άλλες, λιγότερες συχνές αιτίες, είναι το εγκεφαλικό επεισόδιο, η χρήση παράνομων ναρκωτικών ουσιών και πνευμονικές ασθένειες.
Οι μηχανισμοί που οδηγούν στους ξαφνικούς αυτούς θανάτους είναι είτε μηχανικής φύσεως, είτε ηλεκτρικής, με τους δεύτερους να αποτελούν πάνω από το 90% των περιπτώσεων. Τα αθλήματα στα οποία συμβαίνουν συχνότερα οι ξαφνικοί θάνατοι είναι το ποδόσφαιρο, η καλαθοσφαίριση, το τρέξιμο, η ποδηλασία και η κολύμβηση. Η συχνότητα θανάτου από συγκεκριμένο άθλημα οφείλεται στον συνδυασμό της καρδιαγγειακής απαίτησης του και του συνολικού αριθμού των ατόμων που ασχολούνται με αυτό.
Ο προληπτικός έλεγχος των αθλητών για την συμμετοχή τους στην αθλητική διαδικασία σώζει ζωές και όλοι συμφωνούν πως είναι απαραίτητος. Όμως υπάρχει μια σοβαρή διαφωνία μεταξύ των Η.Π.Α. και της Ευρώπης, για το αν θα πρέπει να συμπεριληφθεί στον έλεγχο και το ηλεκτροκαρδιογράφημα. Ο κυριότερος λόγος που επικαλούνται οι Η.Π.Α. στην μη χρήση του ΗΚΓ, είναι το υψηλό κόστος.
Η χρόνια άσκηση, προκαλεί αναδιαμόρφωση της καρδιάς ώστε να ανταποκριθεί στις καρδιαγγειακές απαιτήσεις. Το φαινόμενο αυτό ονομάζεται «αθλητική καρδιά». Αυτή εξαρτάται από την ένταση και τον τύπο της άσκησης, όπου στους αθλητές αντοχής, παρουσιάζεται κυρίως αύξηση της εσωτερικής διαμέτρου της αριστερής κοιλίας και στους αθλητές δύναμης, αύξηση του πάχους του τοιχώματος της αριστερής κοιλίας. Συχνά γίνεται σύγχυση μεταξύ της «αθλητικής» και της παθολογικής καρδιάς καθότι σε ήπιες μορφές της ασθένειας, ορισμένα χαρακτηριστικά όπως το πάχος του τοιχώματος της αριστερής κοιλίας, μπορεί να παρουσιάζουν παρόμοια μεγέθη.
Abstract
Sudden death during sports activities is a relatively rare, yet tragic event, with significant economic and social consequences. According to the existing studies, the annual incidence ranges from 3/100 000 to 1/250 000. The victims are mainly males with a ratio of nine men to one woman. The colored athletes are proportionally more susceptible and have a particular sensitivity to hypertrophic cardiomyopathy. Athletes have a 2.8/1 higher risk to have sudden death than non-athletes. But this is usually due to the undetected underlying cardiovascular disease.
The main cause of sudden cardiac death in individuals older than 35 years by far is the coronary heart disease. With regard to people less than 35 years old, opinions differ, with studies on the U.S. having the first cause the hypertrophic cardiomyopathy, while studies in Italy, the arrhythmogenic right ventricular heart disease. Other common causes of sudden death during exercise, are congenital anomalies of the coronary arteries, Commotio Cordis, myocarditis, diastolic cardio-myopathy, prolapse of the mitral valve, aortic stenosis, aortic rupture, etc. From the non-cardiac sudden deaths, blunt trauma is the most frequent cause and the parts of the body that are more frequently battered are the head and neck. Other, less common causes are stroke, the use of illicit drugs and pulmonary diseases. The mechanisms leading to these sudden deaths are either of mechanical nature or electrical with the latter constituting for over 90% of the cases. The sports in which most frequently occur sudden deaths are football, basketball, running, cycling and swimming. The death rate in each sport is due to the combination of the cardiovascular requirement and the total number of people involved with it.
Screening of athletes for their participation in the athletic procedure saves lives and everyone agrees that it is necessary. But there is a serious disagreement between the U.S. and Europe, whether the screening procedure should include the electrocardiograms. The main reason cited by the U.S. for the non-use of the ECG, is the high cost it requires. Chronic exercise induces the remodeling of the heart in order to respond to the cardiovascular demands. This phenomenon is called 'the athlete’s heart'. This depends on the intensity and type of the exercise, where in endurance athletes, it causes mainly the increasing of the internal diameter of the left ventricle and in power athletes, the increasing of the wall thickness of the left ventricle. There is often confusion between the "athlete’s" and the abnormal heart since in mild forms of the disease, some features such as the thickness of the wall of the left ventricle, can have similar sizes.
